  4.           SUMMARY OF STATE LIVING WILL LEGISLATION

  8. Synopsis: 
  9. To prepare a living will using the State of Utah recommended
  10. form, please select living will preparation from the opening
  11. menu.  Remember that we recommend preparing a "uniform" living
  12. will in addition to the form suggested with by the State of Utah.
  13. Please also consider executing a living will for any State that
  14. you spend more than a few weeks per year, or where you are a
  15. temporary resident.

  26. Relationship: Witnesses may not be related by blood or marriage,
  27. may not be directly responsible for the medical costs of the
  28. declarant, may not be entitled to any part of the declarant's
  29. estate whether under will or intestacy, may not be an agent of 
  30. a hospital in which the declarant is a patient; and must be over
  31. 18 years of age.

  33. Recording of the living will is not provided for by statute

  35. Revocation of the living will may be made physical destruction
  36. of the declaration (by tearing, defacing, burning, etc.); or
  37. by a separate written instrument.  It may be verbally revoked if
  38. a witness to the verbal revocation makes a written statement that
  39. he or she witnessed the revocation
